Grounding 5-4-3-2-1 worksheet

Grounding 5-4-3-2-1 worksheet by a Milton Keynes counsellor: a free PDF to anchor you in the present when anxiety spikes.

Grounding 5-4-3-2-1 is one of the simplest, most reliable tools for interrupting an anxiety spike, a panic surge or a dissociative drift. It anchors you back into the present by walking the five senses in sequence. You name five things you can see, four you can hear, three you can feel, two you can smell, one you can taste. It takes about ninety seconds. It works because it moves the brain’s attention away from catastrophic thought and back into direct sensory input.

This worksheet gives you the exact prompts, in a printable one-page format you can keep on your phone, in your bag or on your desk. Clients I work with often find it most useful in three situations: when a panic feeling is starting to build, when a flashback or dissociative moment pulls them out of the room, or when night-time anxiety is making sleep impossible.

When to use grounding 5-4-3-2-1

Grounding works best as a repeated practice, not only as a crisis tool. Try it in low-stakes moments when you are already reasonably calm, so your nervous system learns the sequence. Then it is available when you actually need it.

  • Rising anxiety or panic.
  • Intrusive memories or flashbacks.
  • Dissociation and feeling far away from yourself.
  • Overwhelm during a difficult conversation.
  • Night-time rumination that is keeping you awake.

When counselling helps alongside

Grounding is a short-term tool. If anxiety or dissociation is a regular feature of your week, counselling can help you work on what is driving the pattern. At Thrive, sessions take place in person in Central Milton Keynes or online across the UK, including Bedford and Northampton. Individual sessions are £55 for 50 minutes. The contact page is the simplest way to start.
