Grief and loss worksheet for making space for a loss that is still shaping your days. It is a short, one-page prompt sheet, not a prescription — grief does not follow a tidy timetable, and this worksheet does not pretend it does.
The worksheet moves gently through four prompts: what you have lost (naming it, in your own words), what has been hardest, what has helped (even in small ways), and what you would like the person, relationship or part of your life you have lost to know. There is space to come back to it over time, because grief changes shape rather than ending.
Download the free one-page PDF below. It is yours to use, print and share freely.

When this worksheet fits
- You are grieving a person who has died and want somewhere to put what is hard to say out loud.
- You are grieving a relationship that has ended, a role, a career, a pregnancy, a home or a version of your future you had expected.
- Someone close to you has had a serious diagnosis and you are grieving while they are still here.
- You are supporting someone else through loss and want a prompt sheet to share.
- You want a simple, unhurried structure to use alone or alongside counselling, revisited as grief shifts.
A note on when to seek more support
Grief is painful but not an illness. If it is overwhelming, if you are finding it hard to function or sleep, or if you are experiencing thoughts of self-harm or suicide, please contact your GP, call NHS 111 and press 2 for the mental health option, or call Samaritans free on 116 123, 24 hours a day. For specialist bereavement support, Cruse Bereavement Support offers a free helpline on 0808 808 1677. In an emergency, call 999 or go to A&E.
When counselling helps alongside
Grief does not need to be fixed. It does, sometimes, need to be witnessed by another person — unhurried, without judgement — so that it has room to move. At Thrive, sessions take place in person in Central Milton Keynes or online across the UK, including Bedford and Northampton. Individual sessions are £55 for 50 minutes. The contact page is the simplest way to start.
