CBT Thought Record Worksheet: Free PDF Download

Free CBT thought record worksheet (PDF). Notice anxious thoughts, test them against evidence and arrive at a balanced, steadier thought.

CBT thought record worksheet for noticing anxious thoughts, testing them against evidence and arriving at a steadier, more balanced thought. It is a short, one-page prompt sheet based on standard cognitive behavioural therapy practice.

The worksheet walks through six steps: the situation, the automatic thought, how strongly you believe it, the feelings and body sensations, the evidence for and against, and a balanced alternative thought with your updated belief rating. There is space to use it on your own, between sessions, or as a quick check-in when anxiety spikes.

When this worksheet fits

  • You notice anxious thoughts spiral quickly and want a structured way to slow them down.
  • You find yourself believing worst-case predictions and want to test them against evidence.
  • You are between counselling sessions and want to practise what you are working on in therapy.
  • You tend to criticise yourself harshly and want to see your thinking on paper, where it is easier to question.
  • You want a simple, repeatable prompt that works for health anxiety, social anxiety, generalised worry or self-critical thinking.

A note on when to seek more support

If anxious or distressing thoughts are overwhelming, if you are struggling to sleep or function, or if you are experiencing thoughts of self-harm or suicide, please contact your GP, call NHS 111 and press 2 for the mental health option, or call Samaritans free on 116 123, 24 hours a day. In an emergency, call 999 or go to A&E.

When counselling helps alongside

A worksheet is a useful prompt but it is not a substitute for working through anxious patterns with another person. Counselling gives you space to understand where the thoughts come from, what keeps them going, and what helps them settle. At Thrive, sessions take place in person in Central Milton Keynes or online across the UK, including Bedford and Northampton. Individual sessions are £55 for 50 minutes. The contact page is the simplest way to start.
